The 1917 Clinic at the University of Alabama at Birmingham (UAB) is seeking a qualified person to provide HIV education, counseling, and linkage to care. The Linkage Retention Coordinator (Program Admin II) under general supervision, will ensure linkage to care, retention in care, and re-engagement activities for newly diagnosed patient, patients at risk of falling out of care, and patients who have fallen out of care. Provides information, support, and assistance with overcoming barriers to care and navigating the healthcare system. Works closely with Social Services Team and community partners to optimize linkage and retention in care for all patients. Handles data management of assigned caseload requiring proficiency with MS Office software, IDX, IMPACT, and other internal systems once trained. Assists with reports as needed.

Key Roles & Responsibilities

  • Links HIV-positive individuals to care and re-engages HIV-positive individuals back into care with appropriate documentation
  • Manages UAB Emergency Department referrals
  • Delivery of HIV Diagnosis and Patient Support:
  • Monitors daily mail/lab reports to identify new HIV diagnosis.
  • Delivers HIV positive diagnosis to patients tested via Routine Universal Screening for HIV Program.
  • Provides support and linkage to HIV Primary Care.
  • Identifies Acute HIV Infections via lab results from Routing Universal Screening for HIV Program.
  • Facilitates Fast Track protocols for immediate referral
  • Acts as a communication liaison with outside resources.
  • Assists clients in selecting a healthcare provider, scheduling initial appointments, and navigating the process of accessing HIV treatment.
  • Identifies and addresses barriers to care such as transportation, insurance, stigma, or financial concerns.
  • Implements strategies to retain patient in care

Knowledge, Skills, Abilities & Work Characteristics

  • Bachelor's degree in public health education, education, social, behavioral or biological sciences or a directly related field
  • This position requires a person who is comfortable communicating with broad populations i.e. persons experiencing homelessness, persons with substance use disorders, persons with stigma, and persons with trauma.
  • Experience with the HIV community
  • Excellent interpersonal skills with the ability to relate to a broad group of people Excellent written and verbal communication skills
  • Experience in providing HIV education
  • Good working knowledge of motivational interviewing and trauma informed care

Hourly Range: $18.35 - $25.00

Qualifications

Education

Bachelor's degree in a related field and one (1) year of related experience required.
