Description

To provide direct and indirect patient care in a clinical area for specific age and patient populations based on the nursing process, UAB Hospital Standards of Nursing Care and UAB Hospital policies and procedures. To orient patients and family members to surrounding environment, acting as a patient advocate. To collect patient assessment data and notify the appropriate staff of potential problems. To evaluate patient's response to care and report findings as appropriate. To document delivery of patient care, effectively respond to emergency situations, meet patient's educational needs and serve as a role model to other members of the patient care team.

Duties and Responsibilities:

  • Provides direct patient care in a clinic environment.
  • Assists in the coordination of clinic research, performs clinical tasks and provides for patient and family educational needs.
  • Thoroughly documents all patient care. This involves: Setting appointments for patients and enter information into appointment database, send schedule to appropriate staff Assisting physician/other nurses/other staff during pediatric and adolescent clinic Documenting care given during clinic on clinic notes Enter all patient contact information including face to face, telephone, referral information, etc. into CAREWare Communicates effectively with applicable patient/study participant populations.
  • Ensure that all patients of Family Clinic are up-to-date on their labs and immunizations. This involves: Reviewing the lab values and immunization status of patients in pre-clinic meeting Reporting (by email) all abnormal lab results to the appropriate physician Ensuring all patient immunizations are up-to-date Assist in maintaining an inventory of necessary lab and vaccine supplies for Family Clinic patients Assists with management of patient flow through the clinic. Collaborates on clinical level regarding physicians' schedules. Triages patients and acts as a liaison with other physicians when case is combined with other disciplines.
  • Participating in quality assurance, educational and any other activities that assist the Family Clinic in reaching its goals and objectives. This involves: Participating in quarterly Quality Improvement meetings and Quality Assurance Projects Monitor HIVQual clinical indicators as outlined by HRSA and other projects that impact the standards of medical care provided to patients and families as determined by Family Clinic management Develop health care protocols, procedures and forms as appropriate in order to improve and standardized care to patients Providing HIV/AIDS education, adherence education, community outreach, counseling, testing, and supportive services Attending relevant training, continuing education, and workshops to maintain licensure, build knowledge base and enhance social work skills Engaging in community networking and collaboration with AIDS service organizations to maintain and enhance knowledge of community resources Performing projects assigned by supervisor/program management, as well as any other tasks to assist the medical care providers and support staff in providing education, outreach, and quality care to those being served
  • Perform other duties as assigned

Salary Range: $17.10 - $25.00

Qualifications

Graduate from an accredited Practical Nursing program required. Work experience may NOT substitute for education requirement. Currently licensed or licensed by endorsement as a licensed practical nurse (LPN) by the Alabama Board of Nursing required. Currently certified with Cardiopulmonary Resuscitation (CPR) required or must be obtained within 30 days of employment.
