Compensation:
Pay Range: $77,640 - $126,170 / year

Job Description:
To lead, supervise and organize rehabilitation and pre-admission case management teams. To monitor and improve the case management process. To liaise with internal and external stakeholders, participate in complex case reviews, manage discharge and post-discharge activities and ensure accurate documentation and compliance with regulations. To foster a collaborative team environment, lead performance initiatives, assist with budget management and formulate departmental policies.
Qualifications
Bachelor's degree in Nursing, rehabilitation related discipline or related field required. Master's degree in a relevant discipline preferred. Five (5) years of experience as a licensed clinical professional, preferably in a rehabilitation case management capacity, required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama State Board of Nursing or licensed as a Speech-Language Pathologist, Occupational Therapist or Physical Therapist by the appropriate State of Alabama Board required. Current Certified Case Manager (CCM) by the Commission for Case Manager Certification (CCMC) required or must be obtained within one (1) year from date of hire into position required.
