Job Description:
To coordinate care services, conduct utilization review and provide case management for Spain Rehabilitation Center patients in a cost effective manner. To orient and support the patient and family to the rehabilitation process. To assess patient’s status. To coordinate and facilitate discharge planning. To effectively transition the patient through the continuum of care.
Qualifications
Bachelor’s degree from an accredited program of Nursing, Occupational Therapy, Physical Therapy or Speech-Language Pathology and three (3) years of related experience in a rehabilitation environment and/or case management required. Rehab case management experience preferred. Work experience may NOT substitute for education requirement. Current licensed or eligible to be licensed as a Speech-Language Pathologist, Occupational Therapist, or Physical Therapist by the appropriate State of Alabama Board required. Currently certified with Cardiopulmonary Resuscitation (CPR) required, or currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Currently certified with Cardiopulmonary Resuscitation (CPR) required.
