CASE MANAGER I-SUBSTANCE ABUSE

General Responsibilities
To provide comprehensive case management, support, and advocacy to individuals possessing low to moderate risk/needs scores for criminogenic behavior/cognition.
To promote the health and wellbeing of individuals and families affected by the history of maladaptive behaviors and cognition, as well as some substance abuse and mental illness.
To administer comprehensive, evidence-based screening and assessment to identify behavioral health needs and other risk factors that may require further intervention.
To develop individualized plans of care that address medical and psychological needs, housing, vocational/educational needs, transportation, substance abuse and social support needs.
To provide case plan monitoring and on-going support through the recovery process and coordinate discharge planning through the development of aftercare discharge plans and linkage with community resources and other services to meet on-going needs.
Key Roles & Responsibilities
Conducts evidence-based screening and assessment to identify patient behavioral health and other social service needs and barriers to disease management.
Develops individualized plans of care for lower risk populations to address lower risk criminogenic, medical, psychological needs, housing, vocational/educational needs, transportation, substance abuse and social support needs.
Provides case plan monitoring and on-going support through the recovery process.
Facilitates referrals for further clinical assessment as indicated.
Coordinates discharge planning through the development of aftercare discharge plans and linkage with community resources and other services to meet on-going needs
Documents patient progress in the clinical record according to established standards.
Implements appropriate evidence-based behavioral health interventions for lower criminogenic risk populations, to address needs identified through initial screening and assessment.
Maintains a working knowledge of available behavioral health and social service providers in the community.
Provides and documents relevant patient progress according to policy.
Updates the assessment and intervention strategies, in accordance to accepted national standards.
Participates in quality improvement initiatives and educational/research activities.
Performs other duties as assigned.
Preferred:
Preferred prior or current CRO certification by the Alabama Office of the Courts.
Familiarity with criminal justice populations, mental health issues, and exposure to risk/needs assessments.
Ability to be a team player.
Comfortable with data entry into software systems.
Familiarity with working in a variety of settings, courtroom, personal office.
Familiarity with the MIDAS software system.
Patient population: Adult 18 years to 60 years
Additional
Following training to facilitate the Certified Ohio Risk Assessment (ORAS)/MIDAS evaluation.
Follow guidelines to facilitate case planning with program participants.
Follow case management guidelines according to the case plan developed from the ORAS evaluation.
Follow Federal Health Information Privacy Guidelines.
Follow guidelines for sending participants for increased mental health evaluation.
MVR NEEDED
Hourly Range: $19.80-$32.15
Qualifications
Education
Bachelor's degree in Social Work, Community Health and Human Services or a related field required.
Work experience may NOT substitute for education requirement.
