Summary: RN CLINICAL CARE COORDINATOR – Sparks Clinic and STEP Clinic: To manage all aspects of the plan of care for a caseload of patients with Down syndrome. To serve as the liaison with the multidisciplinary care team and coordinate care with sub-specialists throughout the health system. To plan care management as achieved through coordination of care with physicians, nursing staff and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. May coordinate patient follow-up clinics and schedule tests. Will facilitate patient pool of calls, referrals and incoming requests as needed. Patients seen through this clinic will be complex in nature and will require education materials, follow up phone calls and additional strategies to ensure patient compliance.
Duties/Responsibilities:
Obtain records on new patients
- Assist in preloading histories
- Medication Reconciliation
- Confirmation of patient visits
- Creation of weekly interdisciplinary schedule.
- Manage planning and organization of multidisciplinary clinic collaboration
Meet patients/ families to establish relationships
- Assist with referrals, med reconciliation, phone messages, scheduling
- Ensure all patient assessments are completed
Follow-up Referrals and social work needs with our assigned UAB social worker
- Manage Daily Pool
Assist in development of template to be standardized across two EMRs
- Bridge patients to Social Work, nutrition, community resources
- Find and create educational resources
- Review chart for sub-specialist notes and visits to coordinate appointments and tests to prevent duplication of services.
Salary Range: $31.53 - $41.53
Qualifications
Associate's degree in Nursing and five (5) years of relevant clinical nursing experience required; Bachelor's degree in Nursing preferred. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required.
