Compensation:
Pay Range: $104,045.00 - $169,075.00 / Yearly

Job Description:

UAB Hospital is recruiting for a Therapeutic Policy Management (TPM) Pharmacist with an emphasis in Hemostatic and Antithrombotic Stewardship.

This position will work in collaboration across pharmacy and medical specialists to treat patients with rare blood disorders including but not limited to hemophilia A, hemophilia B and von-Willebrand disease. In addition, this position will ensure proper use of all blood factor products.

Key responsibilities for the position will include but are not limited to:

· Serve as Co-Chair of the Hemostatic and Antithrombotic (HAT) Stewardship Program

· Serve as Coordinator of the Anticoagulation P&T Subcommittee

· Develop and review Anticoagulation Guidelines for Use

· Provide daily pharmacotherapy review of all hospitalized patients who require factor therapy

· Coordinate inventory management of factor products for both hospital and patient’s own supply

· Assist with other TPM responsibilities as assigned

· Participate in the Non-formulary and Drug Information administrative call program


Preferred Qualities:

· Completion of an ASHP –accredited PGY-1 residency

· Completion of an ASHP- accredited PGY-2 residency in Thrombosis and Hemostasis Management

· Board Certified Pharmacotherapy Specialist (BCPS)

· At least 3 years’ experience in treating patients with blood disorders

Qualifications

Bachelor’s degree in Pharmacy required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Licensed Pharmacist by the Alabama Board of Pharmacy (ALBOP) required.
