To provide patient-focused pharmacy services including monitoring of therapy, interventions, basic pharmacokinetics, education and medication distribution. To support the mission, activities and programs of the division, department and the Hospital.
Job Description:
· Compounds or dispenses outpatient prescriptions or inpatient drug requests in accordance with all legal requirements, hospital policies, and professional judgment. Responds to requests in a timely manner. Ensures that all billing information is accurate on prescription or internal requisitions.
· Provides dispensing activities and pharmaceutical support for staff and patients according to state and federal laws. Reviews prescriptions to ensure accuracy, ascertain needed ingredients and to evaluate suitability. Accurately documents patient's medication history profile; notifying appropriate physician or other healthcare professional as to any issues regarding compliance or medication therapy regimen. Assures responsible distribution of medications for administration in assigned area of coverage.
· Accurately assesses the appropriateness of each medication as it is prescribed based on the patient's diagnosis, condition, allergies, and medication history, informing physicians of potential adverse drug reactions, drug interactions, or alterations that could improve patient outcome. Promotes rational medication therapy by evaluating all orders in the context of current literature, UAB medication evaluations and the UAB Formulary, contacting the physician and making recommendation for changes when appropriate.
· Provides drug information to patients and family members, physicians, pharmacists, nurses and other health care professionals. Shares clinical observations and new information from medical/pharmaceutical literature in order to positively impact the optimal use of medications.
· Actively monitors patient specific response to prescribed therapies; reviewing potential adverse drug reactions and drug interactions to optimize therapy. As needed, re-evaluates prescribed therapy utilizing information gained from patients, patient charts, physician rounds, physician order forms and conversations with other health care professionals, suggesting appropriate changes.
· Supports educational programs of the Division and the department by mentoring pharmacy students preparing for graduation or licensing and by maintaining all required licensure and certification as well as attendance at departmental and hospital programs.
· Performs other duties as assigned.
Qualifications
Bachelor's degree in Pharmacy required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Licensed Pharmacist by the Alabama Board of Pharmacy (ALBOP) required.
